Patrick Troughton is such a brilliant actor. The dialogue doesn't actually have the Doctor calling Zaroff mad, but Troughton makes sure we know that's what the Doctor thinks, simply by the delivery of the dialogue.

Yes, that's an amazing and intense scene; as you say, it's the moment where Troughton works out how to play his Doctor and sticks with it from there.
Reversing the polarity of the neutron flow. I bet that means something. It sounds great.

I really, really like the two stories before this, Power (a real classic) and the Highlanders (Polly's best story and really solid, strong stuff), but Troughton's initial characterisation is a manic character with an keen intelligence behind that. Here it becomes more much underplayed while keeping the keen intelligence.
